This free, live Web event from NEJM Catalyst, hosted by Ochsner Health System, will feature an outstanding slate of experts who will share their experiences, best practices, and frameworks for excellence with you.

The challenges of health care today are different than in the past, and so are the solutions. Through engaging talks and live, moderated Q&A, we will address three pain points shared across the health care continuum: chronic care, centers of excellence, and health IT.
